Abstract
Objective To explore the related factors influencing the recovery of swallowing function in patients with dysphagia after stroke and construct a nomogram prediction model.Methods A total of 190 patients with dysphagia after stroke admitted to the Second Affiliated Hospital of Nanchang University from January 2022 to June 2024 were selected and divided into poor prognosis group(n=72)and good prognosis group(n=118)according to their prognosis.The clinical data of two groups of patients were collected,and Logistic regression analysis was used to analyze the related factors influencing the recovery of swallowing function in the patients.Results There were statistically significant differences in age,National Institutes of Health stroke scale(NIHSS)score,body mass index(BMI),smoking history,the injured side of stroke,and the number of previous strokes between two groups of patients(P<0.05).The areas under the curve for predicting poor prognosis of patients'swallowing function based on age,BMI and NIHSS score were 0.657,0.623 and 0.862 respectively.The results of Logistic regression analysis showed that advanced age,smoking history,left side injury due to stroke,and high NIHSS score were all risk factors affecting the recovery of swallowing function in patients with dysphagia after stroke(P<0.05).A nomogram model was constructed.Its internal validation showed that the C-index was 0.774(0.723-0.798),and the model predicted that the risk threshold for the recovery of swallowing function in patients was>0.04.Conclusion Advanced age,smoking history,left side injury caused by stroke,and NIHSS score are all risk factors affecting the recovery of swallowing function in patients with dysphagia after stroke.The constructed nomogram prediction model has a relatively high clinical value.关键词
