平衡力线手法联合常规推拿治疗早中期膝骨关节炎的研究
Study on the Treatment of Early to Mid-stage Knee Osteoarthritis with Balance-Alignment Manipulation Combined with Conventional Tuina

Abstract
Objective:To observe the therapeutic effect of balance-alignment manipulation combined with conventional Tuina on early to mid-stage knee osteoarthritis(KOA).Methods:A total of 80 patients with early or mid-stage KOA were randomly divided into a treatment group and a control group using a random number table,with 40 cases in each group.The patients in control group were treated with conventional tuina manipulation,30 minutes per session,three times per week.On the basis of the control group's regimen,the patients in treatment group were treated with balance-alignment manipulation,which included tendon-stagnation relieving manipulation and bone-displacement correcting manipulation,also 30 minutes per session,three times per week.Both groups were treated for one course of 4 weeks.The efficacy was evaluated after one treatment course.The outcome measures included the visual analogue scale(VAS)score,the Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)score,mean power frequency(MPF)of surface electromyography(sEMG)value of the rectus femoris,vastus lateralis,and vastus medialis muscles.Results:In the treatment group,1 case was clinical controlled,29 cases showed marked effect,9 cases showed moderate effect,and 1 case showed no effect,with a total clinically controlled rate and markedly effective rate of 75.00%(30/40).In the control group,0 case was clinical controlled,14 cases showed marked effect,25 cases showed moderate effect,and 1 case showed no effect,with a total clinically controlled rate and markedly effective rate of 35.00%(14/40).There was significant difference between the efficacy of treatment group and control group(P<0.01).After treatment,VAS score and WOMAC scores(including scores of pain,stiffness,physical function,and total score)in both groups redused significantly(P<0.01),and the scores of treatment group were significantly lower than the scores in control group(P<0.05 or P<0.01).The values sEMG and MPF increased significantly in both groups(P<0.01),and the values in treatment group were significantly higher than those of the control group(P<0.05 or P<0.01).Conclusion:Balance-alignment manipulation combined with conventional Tuina exerts certain effect in treating early and mid-stage KOA.It can relief pain,improve knee joint function,and enhance quadriceps muscle strength.关键词
