减重代谢手术同期联合行腔镜甲状腺癌根治术的安全性及短期疗效分析
Analysis of the safety and short-term efficacy of simultaneous metabolic bariatric surgery combined with endoscopic radical thyroidectomy

Abstract
Objective:To investigate the safety and efficacy of simultaneous metabolic bariatric surgery combined with endo-scopic radical thyroidectomy.Methods:A single-center retrospective cohort study was conducted.According to the inclusion and exclu-sion criteria,the clinical data of 23 patients who underwent simultaneous metabolic bariatric surgery and endoscopic radical thyroidec-tomy(combined group)and 98 patients who underwent endoscopic radical thyroidectomy alone(simple group)from Jan.2018 to Nov.2024 were collected.The operative time,intraoperative cervical blood loss,number of dissected lymph nodes,indwelling time of cervical drainage tube,postoperative cervical drainage volume,postoperative hospital stay,hospitalization cost,and early postoperative complications were compared between the two groups.The weight loss effect and metabolic improvement of the combined group were evaluated at 1 month postoperatively.Results:All operations were successfully completed under endoscopy.Compared with the simple group,the combined group had significantly longer operative time,postoperative hospital stay and indwelling time of cervical drainage tube,as well as higher postoperative cervical drainage volume and hospitalization cost,with statistically significant differences(P＜0.05).There were no significant differences between the two groups in intraoperative cervical blood loss,number of dissected lymph nodes,or the incidence and severity of total complications within 1 month after surgery(P＞0.05).The incidence of gastroesophageal reflux in the combined group was significantly higher than that in the simple group(P＜0.05).In the combined group,BMI,mean arte-rial pressure,triglyceride,fasting blood glucose,and glycosylated hemoglobin at 1 month after surgery were significantly lower than those before surgery(P＜0.05).Conclusions:For obese patients complicated with thyroid cancer who meet the screening criteria,si-multaneous combined surgery is safe and feasible,with significant short-term weight loss and metabolic improvement effects,but it will increase operative time,hospital stay and hospitalization cost.关键词
