Discharge planning for elderly patients with fragility hip fractures:a summary of best evidence
摘要
Abstract
Objective:To summarize the best evidence regarding discharge planning for elderly patients with fragility hip fractures in China and abroad,thereby providing an evidence-based foundation for establishing standardized discharge planning protocols and clinical practices.Methods:Literature on discharge planning for elderly patients with fragility hip fractures were retrieved from UpToDate,BMJ Best Practice,Guidelines International Network(GIN),Medlive,Cochrane Library,PubMed,FMRS,Web of Science,CNKI,Chinese Medical Journals,CBM,Wanfang,and VIP Database,including clinical decision support tools,guidelines,systematic reviews,and expert consensus statements.The research period is from database inception to October 31,2025.Results:A total of 18 documents were included,comprising 1 clinical decision support tool,5 guidelines,4 systematic reviews,and 8 expert consensus statements.Twenty-five evidence items were summarized,categorized into five themes:screening and assessment of target patients,formulation of discharge plans,in-hospital implementation of discharge plans,pre-discharge preparation,and follow-up evaluation of outcomes.Conclusion:This study summarizes the best evidence for discharge planning for elderly patients with fragility hip fractures.It is recommended that healthcare professionals,during the subsequent process of evidence translation,combine this evidence with clinical realities to scientifically formulate and strictly implement discharge plans,thereby facilitating safe patient discharge and further enhancing the efficiency and quality of healthcare services.关键词
