和血除湿汤联合针刺治疗血虚风燥型湿疹的随机对照研究
A randomized controlled study of Hexue Chushi Decoction combined with acupuncture in the treatment of blood-deficiency and wind-dryness type eczema

Abstract
Objective To investigate the clinical efficacy,safety,and regulatory effects on serum IL-4,IFN-γ,and TSLP of Hexue Chushi Decoction combined with acupuncture in the treatment of blood-deficiency and wind-dryness type eczema.Methods A total of 74 patients with blood-deficiency and wind-dryness type eczema were enrolled and randomly assigned to a treatment group and a control group using a random number table method.The control group received oral administration of Hexue Chushi Decoction,while the treatment group received additional acupuncture therapy.Acupoints included Shanzhong(CV17),Quchi(LI11),Tianshu(ST25),Xuehai(SP10),Zusanli(ST36),Yinlingquan(SP9),Fengshi(GB31),Feishu(BL13),Dachangshu(BL25),Geshu(BL17),and Sanyinjiao(SP6).Acupuncture was performed twice a week for 30 minutes per session,with a total intervention period of 4 weeks.Evaluation parameters compared between the two groups before and after treatment included the Eczema Area and Severity Index(EASI),Visual Analogue Scale(VAS)for pruritus,Dermatology Life Quality Index(DLQI),Pittsburgh Sleep Quality Index(PSQI),Hospital Anxiety and Depression Scale(HADS),as well as serum levels of inflammatory factors IL-4,IFN-γ,and TSLP,and adverse reactions were monitored.Results The total effective rate in the treatment group was 83.9%,which was significantly higher than the 63.9%in the control group(P<0.05).After 4 weeks of treatment,the EASI score,pruritus VAS score,and HADS score in the treatment group were significantly lower than those in the control group(P<0.05).However,there was no statistically significant difference between the two groups in the improvement of DLQI and PSQI(P>0.05).Laboratory tests showed that the serum IL-4 level in the treatment group was significantly lower than that in the control group,and the IFN-γ level was significantly higher than that in the control group(P<0.05),while the difference in TSLP between the groups was not statistically significant(P>0.05).No serious adverse reactions occurred in either group during the treatment,indicating good safety.Conclusion The combination of Hexue Chushi Decoction and acupuncture can further improve the clinical symptoms of skin lesions,alleviate pruritus,and ameliorate anxiety and depression in patients with blood-deficiency and wind-dryness type eczema.The underlying mechanism may be related to the downregulation of serum IL-4 levels and the upregulation of IFN-γ levels,and this comprehensive therapy demonstrates a favorable safety profile.关键词
