Abstract
Objective:To analyze the prognostic value of serum lactate clearance rate(LCR)combined with hemoglo-bin(HB)/albumin(ALB)in pregnant women with hemorrhagic shock(THS).Methods:72 THS pregnant women admitted to Dongguan Maternal and Child Health Hospital from January 2020 to December 2024 were divid-ed into two groups according to their acute physiological and chronic health status-Ⅱ(APACHEⅡ)score:good prognosis group(n=61,APACHEⅡ score≤15)and poor prognosis group(n=11,APACHEⅡ score>15).The demographic data,LCR and HB/ALB of the two groups were determined by single factor,and the high-risk fac-tors of poor prognosis of THS pregnant women were determined by multiple factors,so as to explore the predictive value of joint prediction of LCR and HB/ALB on the prognosis of THS pregnant women.Results:Among 72 preg-nant women with THS,11 cases(15.28%)had poor prognosis within 5 days,and 61 cases(84.72%)had good prognosis.Univariate analysis showed that only LCR and HB/ALB were associated with poor prognosis of THS pregnant women(P<0.05).Multivariate Logistic regression analysis showed that low LCR(OR=12.175,95%CI=6.371-41.834)and high HB/ALB(OR=10.569,95%CI=5.735-36.975)were the risk factors for poor prognosis of THS pregnant women(P<0.05).ROC showed that the AUC of joint prediction(95%CI=0.879-1.000)was higher than that of single prediction(95%CI=0.683-0.971)(P<0.05).Conclusion:LCR、HB/ALB combination can effectively predict the prognosis of THS pregnant women,which has clinical reference and promotion value.关键词
