高频线阵探头联合经直肠环扫探头超声检查诊断肛周脓肿的临床价值分析

Abstract
Objective To explore the clinical value of ultrasonography using high-frequency linear probe combined with transrectal rotational scanning probe for the diagnosis of perianal abscess.Methods A total of 90 patients with suspected perianal abscess admitted to our hospital from August 2021 to September 2024 were enrolled,and all patients underwent ultrasonography with the combined use of high-frequency linear probe and transrectal rotational scanning probe.Taking surgical findings as the gold standard for diagnosis,we analyzed the imaging results and lesion detection rates of perianal abscess with different examination methods.In addi-tion,the diagnostic value of each examination method and its consistency with the gold standard were evalu-ated.Results The combined examination yielded higher total lesion detection rate,more single lesions and multiple lesions detected compared with ultrasonography using high-frequency linear probe alone or transrectal rotational scanning probe alone(P<0.05).No statistically significant difference was observed in the number of communicating lesions detected among different examination methods(P>0.05).Statistically significant dif-ferences were found in the detection rates of subcutaneous perianal abscess,submucous rectal abscess and ischiorectal fossa abscess across different examination modalities(P<0.05).By contrast,there were no signifi-cant differences in detection rates of pelvirectal fossa abscess,retrorectal space abscess and fistulas among differ-ent examination methods(P>0.05).The combined examination exhibited significantly higher sensitivity,accuracy and negative predictive value than single-probe examinations(P<0.05),while no inter-group differ-ences were noted in specificity and positive predictive value(P>0.05).Consistency test revealed excellent diagnostic consistency of the combined examination for perianal abscess(kappa=0.911,P=0.000).Con-clusion Ultrasonography combining high-frequency linear probe and transrectal rotational scanning probe pres-ents prominent clinical value for perianal abscess.It effectively increases the number of detectable lesions with high diagnostic accuracy and favorable diagnostic consistency.This examination can provide reliable evidence for clinical diagnosis and treatment formulation,and is worthy of widespread clinical application.关键词
