唇腭裂患者口腔修复后颌面部美观度及其术后并发症影响因素的分析

Abstract
Objective To explore the maxillofacial aesthetic degree of patients with cleft lip and palate after oral repair and analyze the influencing factors of postoperative morbidity.Methods A retrospective analysis was conducted on the data of 86patients with cleft lip and palate admitted to the hospital from January 2021 to June 2025.Among them,there were 53 patients with complete cleft lip and palate and 33 patients with incomplete cleft lip and palate.These patients were divided into the control group(49 cases,no complications after the operation)and the study group(37 cases,complications after the operation)based on whether complications occurred after the operation.Three months after the last operation,all patients selected the Asher-McDade Nasolabial Evaluation Scale to assess the aesthetic appearance of the maxillofacial region,and the aesthetic appearance of the maxillofacial region in patients with cleft lip and palate of different severity was analyzed.The postoperative morbidity conditions such as infection,occlusal disorder and secondary deformity were statistically analyzed.Various data of the patients were collected.Univariate and multivariate Logistic regression analyses were conducted to determine the influencing factors of postoperative morbidity in the patients.Results The score of Ashir-McDade nasolabial evaluation scale at 3 months after surgery was(2.25±0.47)points.Patients with complete cleft lip and palate demonstrated a significantly lower Ashir-McDade nasolabial evaluation scale score than patients with incomplete cleft lip and palate(P＜0.05).There were statistically significant differences between the two groups of patients in terms of age,types of cleft lip and palate,operation time,malnutrition,and frequency of oral hygiene(P＜0.05).Two cases experienced combined complications.Univariate and multivariate Logistic regression analyses indicated that age(OR=0.730),cleft lip and palate type(OR=1.548),surgical time(OR=1.677)and oral cleaning frequency(OR=0.663)were identified as influencing factors of postoperative morbidity(P＜0.05).Conclusion The maxillofacial aesthetic degree after oral repair in patients with cleft lip and palate is associated with the type of cleft lip and palate.Age,type of cleft lip and palate,surgical time and oral cleaning frequency are the influencing factors of postoperative morbidity.Clinically,it is necessary to formulate appropriate repair regimen according to the above factors,and strengthen postoperative management so as to enhance the therapeutic effect.关键词
