医防融合视角下城乡居民医保对农村居民疾病预防行为的影响:基于CHARLS数据的实证研究

Abstract
Objective To analyze the impact of the Urban-Rural Resident Basic Medical Insurance on disease prevention be-haviors of Chinese rural residents from the perspective of the integration of medicine and prevention,so as to provide references for further promoting the integration of medicine and prevention and optimizing the basic medical insurance system.Methods Using da-ta from the China Health and Retirement Longitudinal Study from 2013 to 2020,22654 rural residents were enrolled in the study.Using the progressive difference-in-differences model,the impacts,mechanisms and heterogeneity of the Urban-Rural Resi-dent Basic Medical Insurance on disease prevention behaviors of Chinese rural residents were empirically examined.Results In terms of adopting healthy lifestyles,the Urban-Rural Resident Basic Medical Insurance exerted no significant impacts on smok-ing,drinking,or exercising of these rural residents.In terms of using preventive medical services,the insurance significantly pro-moted the likelihood of rural residents'utilization of physical examination services(P＜0.01),while it showed no significant effect on the number of examination items.The promotion effect was more obvious among rural residents with middle-and high-education levels and those in middle-and high-income groups.Health information access expansion(P＜0.05)and healthcare utilization experience accumulation(P＜0.01)served as key mechanisms underpin the promotion impact.Conclusion Urban-Rural Resident Basic Medical Insurance can promote the disease prevention behaviors of Chinese rural residents to a certain ex-tent.Therefore,in the future,efforts should be made to expand the scope of medical insurance coverage and optimize payment methods,enhance the accessibility of health information and the convenience of preventive medical services,provide greater policy support for vulnerable groups,thus guiding rural residents in disease prevention.关键词
