半夏厚朴汤联合腹腔镜胃底折叠术治疗胃食管反流病临床观察
Clinical efficacy of Banxia Houpo Decoction combined with laparoscopic fundoplication for gastroesophageal reflux disease

Abstract
Objective To investigate the clinical efficacy of Banxia Houpo Decoction combined with laparoscopic fundoplication for gastroesophageal reflux disease(GERD).Methods A total of 70 patients with GERD admitted to the First Department of Surgery of Hebei Provincial Hospital of Traditional Chinese Medicine from June 2022 to June 2024 were enrolled and randomly assigned to a treatment group and a control group,with 35 patients in each group.All patients underwent the"3-1 technique"procedure,namely,the four-point esophageal hiatal approach with incomplete opening of the esophageal hiatus and a 180° anterior fundoplication.After surgery,the control group received conventional treatment,while the treatment group received Banxia Houpo Decoction in addition to conventional treatment for 1 month.The reflux symptom index(RSI),Gastroesophageal Reflux Disease Questionnaire(GerdQ),Reflux Disease Questionnaire(RDQ),lower esophageal sphincter(LES)resting pressure,and DeMeester score on 24-hour esophageal pH monitoring were compared before surgery and at 12 months after surgery.Results All 70 patients with GERD successfully completed surgery;no deaths or conversions to open surgery occurred.No short-term severe complications such as intra-abdominal hemorrhage,intra-abdominal infection,or gastrointestinal perforation were observed postoperatively.During the 12-month follow-up,no patient developed dysphagia.At 12 months after surgery,RSI,GerdQ,and DeMeester scores decreased in both groups(P<0.05),and were lower in the treatment group than in the control group(P<0.05).At 12 months after surgery,RDQ scores decreased in both groups(P<0.05),and LES resting pressure increased significantly(P<0.05);however,no statistically significant differences were found between the treatment and control groups(P>0.05).Conclusion The"3-1 technique"is safe and feasible for the treatment of GERD,and Banxia Houpo Decoction may further improve postoperative clinical efficacy.关键词
