基于奥马哈问题分类系统的肠造口术后患者需求指标体系的构建
Construction of a demand index system for patients after enterostomy based on the Omaha Problem Classification System

Abstract
Objective To systematically construct demand indicators for enterostomy patients across four dimensions(envi-ronment,psychosocial,physiological,and health behavior)using the Omaha Problem Classification System as a framework,providing a scientific basis for enterostomy management practice.Methods Databases including PubMed,CINAHL,EM-base,Web of Science,The Cochrane Library,Wanfang Data,and CNKI were searched.Referring to the structure of the Omaha Problem Classification System,a preliminary demand index system for post-enterostomy patients was developed through evidence-based research and expert interviews.The Delphi method was used to screen evaluation indicators,and the analytic hierarchy process was applied to assign weights to the indicators.Results A total of 15 literatures were included,and 16 experts were selected.The authority coefficients of the two rounds of expert consultation were 0.9 and 0.928,respec-tively,both greater than 0.80.The coefficient of variation in the two rounds were 0.11-0.29 and 0.05-0.22,respectively.After two rounds of consultation,the Kendall's W values for first-level,second-level,and third-level indicators were 0.40,0.36,and 0.56,respectively.The Kendall's concordance coefficients of indicators at all levels showed statistically significant differences(P＜0.001).The final demand index system included 4 first-level indicators,15 second-level indicators,and 39 third-level indicators.Conclusion The demand index system for post-enterostomy patients constructed based on the Omaha Problem Classification System is highly scientific and practical,which can provide reference for clinical nurses in conducting ostomy management work.关键词
