老年人群应激性高血糖比值与血压升高的相关性:基于NHANES的横断面研究
Association between stress hyperglycemia ratio and elevated blood pressure in elderly population:a cross-sectional study based on NHANES

Abstract
Objective To explore the association between stress hyperglycemia ratio(SHR)and elevated blood pressure(EBP).Methods Based on the data of 4 571 American participants aged over 60 years from the National Health and Nutrition Examination Survey(NHANES)database in the United States from 1999 to 2016,multivariate Logistic regression and restricted cubic spline(RCS)analysis were applied to investigate the linear and non-linear associations between SHR and EBP,and thresh-old effect analysis was performed to identify the inflection point.The receiver operating characteristic(ROC)curve was used to evaluate the diagnostic performance of SHR for EBP.Results Multivariate Logistic regression analysis showed that SHR was positively correlated with the risk of EBP in both the unadjusted and fully adjusted multivariate models.RCS analysis based on model 3 revealed a significant U-shaped non-linear relationship between SHR and EBP(Pfor non-linearity＜0.001),with a threshold effect observed(threshold SHR=0.869,likelihood ratio test P＜0.001).Stratified anal-ysis indicated that the association between SHR and EBP was not statistically significant when SHR＜0.869(OR=0.33,95％CI,0.07 to 1.68),while a significant positive correlation was found be-tween SHR and EBP when SHR≥0.869(OR=8.06,95％CI,3.71 to 17.55,P＜0.001).The single linear model demonstrated a positive association between SHR and EBP(OR=2.94,95％CI,1.82 to 4.73,P＜0.001).The ROC curve showed that the area under the curve(AUC)of SHR for discriminating EBP was 0.67.Conclusion In the population aged over 60 years,SHR has an independent association with EBP,which can provide a reference for the identification of abnormal blood pressure in the elderly population.关键词
