Synergistic effects of Swanson's theory of caring combined with Benson's relaxation technique on fatigue-sleep disturbance symptom cluster and comfort in elderly patients with peritoneal dialysis

Abstract
Objective To investigate the synergistic intervention effects of Swanson's theory of caring combined with Benson's relaxation technique(BRT)on the fatigue-sleep disturbance symptom cluster and comfort level in elderly patients with peritoneal dialysis(PD).Methods A randomized controlled trial was conducted.A total of 108 elderly PD patients were randomly divided into study group(n=54)and control group(n=54).The control group received routine PD treatment and nursing care,while the study group received a 12-week intervention combining Swanson's theory of caring with BRT on the basis of routine care.The Piper Fatigue Scale-Revised(PFS-R),the Pitts-burgh Sleep Quality Index(PSQI),and PD Patient Comfort Scale were used to assess fatigue,sleep quality,andcomfort levels in both groups before and after the intervention.Pearson correlation analysis was performed to explore the associations among fatigue,sleep quality,and comfort.Results After the intervention,the study group had significantly lower mean PFS-R score[(2.57±0.91)versus(3.93±2.03)]and PSQI total score[(8.66±2.84)versus(12.13±3.89)]as well as a signif-icantly higher mean Comfort Scale total score[(78.42±4.67)versus(61.38±4.65)]than the control group(P＜0.05).The mean differences(MD)and 95％confidence intervals(CI)be-tween groups were as follows:MD=-1.36,95％CI,-2.01 to-0.71(PFS-R score);MD=-3.47,95％CI,-4.72 to-2.22(PSQI total score);MD=17.04,95％CI,15.01 to 19.07(Comfort Scale total score).Pearson correlation analysis revealed that fatigue was significantly posi-tively correlated with sleep quality(r=0.65,P＜0.01),and both fatigue and sleep quality were significantly negatively correlated with comfort(r=-0.72 and-0.58,respectively;P＜0.01).Conclusion The synergistic intervention of Swanson's theory of caring combined with BRT can ef-fectively break the vicious cycle of"fatigue-sleep disturbance"in elderly PD patients and improve comfort from physiological,psychological,and social dimensions,with the advantages of low cost and high compliance.关键词
