Targeted surveillance analysis of methicillin-resistant Staphylococcus aureus infections in a tertiary hospital in 2025
摘要
Abstract
Objective To monitor the nosocomial infection status of methicillin-resistant Staphylococcus aureus(MRSA)in our hospital in 2025,clarify its epidemiological characteristics and transmission risks, and provide evidence for targeted isolation strategies and standardized antimicrobial administration in clinical practice. Methods MRSA isolates and corresponding clinical data of infected patients were collected from our hospital in 2025.Bacterial identification and antimicrobial susceptibility testing were performed using the Autof MS600 automatic microbial mass spectrometry identification system and VITEK2-compact microbial identification and drug susceptibility analyzer. The interpretation of susceptibility results was conducted in accordance with the 2026 Clinical and Laboratory Standards Institute(CLSI)guidelines.MRSA-infected patients were divided into the treatment group and the control group. The treatment group included patients with hospital-acquired infections(HA),while the control group included patients with community-acquired infections(CA).The two groups were compared in terms of population distribution, specimen sources, specimen composition and antimicrobial resistance profiles. Results The overall detection rate of MRSA infection was 24.64%(138 cases/560 cases).A total of 138 patients with MRSA infection were enrolled in the study, including 75 patients with 75 MRSA strains in the treatment group and 63 patients with 63 MRSA strains in the control group.Stratified analysis showed that MRSA detection was more common in males, accounting for 63.77%of all cases.Adults aged 18 to 65 years were the dominant population with MRSA infection, occupying 47.83%of the total cases.Respiratory system infection was the most common infection site of MRSA,with a proportion of 44.20%(61 strains/138 strains).General antimicrobial susceptibility results indicated that all MRSA isolates were completely susceptible to tigecycline, teicoplanin, vancomycin, ceftaroline and linezolid. The resistance rates of MRSA isolates to levofloxacin in the treatment group and control group were 34.67%(26 strains/75 strains)and 11.11%(7 strains/63 strains),respectively, and the difference was statistically significant(P<0.05).Subgroup analysis based on specimen types revealed significant differences in resistance rates between the two groups. In pus specimens, the clindamycin resistance rate of isolates in the control group was 100.00%(19 strains/19 strains),which was significantly higher than 79.17%(19 strains/24 strains)in the treatment group(P<0.05).For ciprofloxacin, the resistance rate of MRSA isolates in the treatment group was 41.67%(10 strains/24 strains),which was markedly higher than 5.26%(1 strains/19 strains)in the control group(P<0.05).In sputum specimens, the levofloxacin resistance rate of MRSA isolates in the treatment group was 36.67%(11 strains/30 strains),significantly higher than 9.68%(3 strains/31 strains)in the control group(P<0.05).Conclusion Significant differences in the resistance rates to partial quinolones and lincosamides exist between HA-MRSA and CA-MRSA strains. These findings have important clinical implications for the management of patients with MRSA infection. It is necessary to optimize antimicrobial stewardship based on local antimicrobial resistance surveillance data, implement standardized infection control policies, reduce the selective pressure of antibiotics on Staphylococcus aureus, and minimize the risk factors associated with MRSA infection and transmission关键词
