Abstract
Objective To explore the association between health-related quality of life(HRQoL)and their treatment decisions and prognosis in patients with advanced gastric cancer.Method This study retrospectively selected 92 patients with advanced gastric cancer who were admitted to our hospital from October 2023 to May 2025.Baseline HRQoL was assessed using the European Organisation for Research and Treatment of Cancer quality of life questionnaire-core 30(EORTC QLQ-C30)and the gastric cancer-specific module(EORTC QLQ-STO22).The patients were divided into an active treatment group(n=68)and a best supportive care group(n=24)based on their treatment decisions.Follow-up was conducted until December 2025,and the overall survival(OS)and progression-free survival(PFS)were recorded for both groups.Survival analysis was performed using the Kaplan-Meier method and Log-rank test,and multivariate analysis was conducted using the Cox proportional hazards model.Result By May 2025,the median follow-up time was 14.3 months.The median OS of patients in the high baseline QLQ-C30 score group(≥50 points)was significantly longer than that of the low score group(＜50 points)(P＜0.05).Multivariate Cox regression showed that the baseline QLQ-C30 score(HR=0.974,95%CI:0.961-0.987,P＜0.05)and treatment decision(active treatment vs supportive treatment,HR=0.532,95%CI:0.325-0.871,P=0.012)were independent predictors of OS.Conclusion The baseline HRQoL of advanced gastric cancer patients is closely related to their treatment decisions and prognosis.Higher HRQoL scores and active treatment decisions are associated with improved the survival outcomes.The clinical decision-making process should comprehensively evaluate the HRQoL score of patients to achieve individualized treatment.关键词
