摘要
Abstract
Objective To explore the current situation of financial toxicity(FT)in cancer radiotherapy patients and its influencing factors, and to analyze its correlation with quality of life.Method The 110 cancer patients who received treatment in the radiotherapy department of our hospital from January 2023 to December 2025 were enrolled.A retrospective study was conducted using a general information questionnaire, the comprehensive score for financial toxicity(COST)scale, and the European Organization for Research and Treatment of Cancer quality of life questionnaire-core 30(QLQ-C30).Multivariate linear regression analysis and Pearson correlation analysis were used.Result The total COST score of 110 radiotherapy patients was 23.46±7.82(range 6-40).According to the COST grading standard,59 cases(53.64%)had no/mild FT(≥26 points),41 cases(37.27%)had moderate FT(14-25 points),and 10 cases(9.09%)had severe FT(≤13 points),among which 46.36%were patients with moderate or above FT;Multivariate linear regression analysis showed that family monthly income, working status, and medical payment method were independent influencing factors of FT in cancer radiotherapy patients, among which the standardized regression coefficient of family monthly income was the largest, and it was the most influential factor on FT;Pearson correlation analysis showed that the COST score was significantly correlated with the scores of multiple dimensions of the QLQ-C30.The COST score was significantly positively correlated with overall health status, physical function, role function, emotional function, cognitive function, and social function(P<0.01);The COST score was significantly negatively correlated with fatigue, pain, insomnia, and economic hardship items in the symptom domain(P<0.01),and there was no statistical significance in the correlation with nausea, dyspnea, constipation, and diarrhea(P>0.05).As the degree of economic toxicity increased, the overall health status of patients showed a significant downward trend. Conclusion Economic toxicity in cancer radiotherapy patients is relatively common, and the income level, employment status, and insurance methods of patients are closely related to economic toxicity.Economic toxicity is closely related to quality of life, and the heavier the economic burden, the worse the quality of life of patients. It is important to pay attention to the early identification of high-risk groups, carry out targeted intervention and guidance to alleviate the economic toxicity of patients and improve their quality of life.关键词
