Abstract
Objective To evaluate the association between cognitive function level and social participation restriction in patients with glioma within 6 months after surgery based on the Montreal cognitive assessment scale(MoCA)and the impact on participation and autonomy(IPA)questionnaire.Method The clinical data of 146 patients with glioma admitted to our hospital from March 2021 to March 2025 were studied retrospectively. The incidence of cognitive dysfunction within 6 months after surgery was analyzed, and the differences in MoCA scores among patients with different clinical data were compared.Pearson correlation analysis was used to analyze the correlation between MoCA scores and IPA scores, and multiple linear regression analysis was used to investigate the relevant factors affecting cognitive impairment in glioma patients within 6 months after surgery.Result The mean MoCA score of 146 patients was(22.45±3.12)points,and the incidence of cognitive dysfunction was 68.49%(100/146).The dimensions with lower MoCA scores were delayed recall and visuospatial/executive function. The total mean IPA score of 146 patients was(1.85±0.68)points,and the dimensions with higher IPA scores were social life and autonomy outdoors.Pearson correlation analysis showed that the MoCA total score was significantly negatively correlated with the IPA total mean score(r=-0.582,P<0.05).Among the dimensions, the visuospatial/executive function of the MoCA scale showed the strongest negative correlation with the autonomy outdoors(r=-0.512)and social life(r=-0.465)dimensions of the IPA scale;delayed recall was mainly negatively correlated with the family role dimension(r=-0.445).With the total MoCA score as the dependent variable, multiple linear regression analysis revealed that age,WHO pathological grade, tumor location, number of antiepileptic drugs(AEDs)types, and IPA were independently and negatively correlated with the total MoCA score, with IPA having the most significant influence(β=-0.385).Years of education were independently and positively correlated with the total MoCA score. The regression equation yielded R2=0.612,adjusted R2=0.598,F=24.561,P<0.001.Conclusion The incidence of cognitive dysfunction is high within six months after glioma surgery and is closely associated with numerous factors, including age, pathological grade, tumor location, number of AED types,IPA,and years of education.Among these,IPA shows the strongest association with patients'cognitive function.关键词
