改良床上脚踏车运动对AECOPD合并呼吸衰竭患者的疗效和安全性影响
Efficacy and safety of a modified bedside cycling program in patients with AECOPD complicated by respiratory failure

Abstract
Objective To observe the therapeutic effect of using non-invasive ventilator combined with modified bed-mounted pedal exercise in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with respiratory failure.Methods Retrospectively collected medical records of 160 patients with AECOPD complicated by respiratory failure who received treatment at the First People's Hospital of Lin'an District,Hangzhou from January 2023 to December 2024.The patients were divided into observation group and control group based on different treatment methods,with 80 cases in each group.Control group intermittently used non-invasive ventilation(NIV)treatment,while observation group received combined modified bedside cycling exercise therapy.Compare the pulmonary function,blood gas parameters,daily living activity capacity,muscle strength of both upper and lower limbs,and adverse reactions between the two groups of patients.Results After treatment,the partial pressure of carbon dioxide(PaCO2)levels in both groups of patients decreased compared to pre-treatment levels.The forced expiratory volume in one second(FEV1),peak expiratory flow(PEF),FEV1/forced voice channel(FVC),partial pressure of oxygen(PaO2)levels,and Barthel index all increased compared to pre-treatment values.Moreover,observation group exhibited lower PaCO2 levels,shorter non-invasive ventilation duration,and shorter hospital stay compared to control group,while demonstrating higher values for PEF,FEV1,FEV1/FVC,PaO2,and Barthel Index(P<0.05).The tracheal intubation rate in observation group was lower than that in control group(P<0.05).Conclusion NIV combined modified bed-mounted exercise bike program can improve the lung function and blood gas indicators of patients with AECOPD and respiratory failure,reduce the duration of NIV and hospital stay,and enhance the muscle strength of both upper and lower limb groups.关键词
