Abstract
Objective To investigate the differences in clinical,endoscopic,and pathological features between early cardia cancer and early non-cardia cancer,and to identify independent predictive factors for early cardia cancer.Methods A retrospective analysis was conducted on 239 patients with early gastric cancer who underwent endoscopic submucosal dissection at People's Hospital of Xinjiang Uygur Autonomous Region from January 2018 to December 2024.Based on tumor location,patients were divided into early cardia cancer group(n=52)and early non-cardia cancer group(n=187).The clinical,endoscopic,and pathological features of two groups were compared.Multivariate Logistic regression analysis was performed,and the predictive efficacy of pepsinogen-Ⅱ,platelet count,albumin,alpha-fetoprotein(AFP),carbohydrate antigen 125(CA-125),and their combination for early cardia cancer was evaluated using receiver operating characteristic(ROC)curve.Results Multivariate Logistic regression analysis identified reflux esophagitis,albumin,platelet count,pepsinogen-Ⅱ,and AFP as independent factors associated with early cardia cancer.Among them,reflux esophagitis,pepsinogen-Ⅱ,and platelet count were risk factors,while albumin,AFP,and CA-125 were protective factors.ROC curve analysis showed limited predictive ability for individual indicators.However,the combination of platelet count,CA-125,and albumin achieved an area under the curve of 0.73(95%CI:0.65-0.80),with both sensitivity and specificity approximately 71%.Conclusion Early cardia cancer and early non-cardia cancer exhibit distinct clinicopathological features.Independent predictive factors for early cardia cancer include pepsinogen-Ⅱ,platelet count,AFP,CA-125,albumin,and reflux esophagitis.Combined detection of AFP,CA-125,and albumin may assist in the identification and risk assessment of early cardia cancer.关键词
