Abstract
Objective To investigate the efficacy and safety of controllable negative pressure suction sheath in retrograde in-trarenal surgery(RIRS)for renal calculi.Methods A total of 116 patients with renal calculi who underwent RIRS were enrolled and randomly divided into an observation group and a control group using a random number table,with 58 cases in each group.The observation group received RIRS assisted by a controllable negative pressure suction sheath,while the control group was trea-ted with a conventional soft ureteroscope sheath.The stone clearance rate,clinical indices(operative time,hospital stay,intrao-perative blood loss,and surgical cost),postoperative comfort[assessed by Bruggrmann comfort scale(BCS)at 12 h postopera-tively],pain intensity[assessed by visual analogue scale(VAS)at 4 weeks postoperatively],infection markers[procalcitonin(PCT),white blood cell count(WBC),and C-reactive protein(CRP)]before and 4 weeks after surgery,and incidence of postoperative adverse reactions were compared between the two groups.Results The stone clearance rate in the observation group was 93.10%(54/58),significantly higher than79.31%(46/58)in the control group(P<0.05).The observation group had shorter operative time and hospital stay,less intraoperative blood loss,and lower surgical cost than the control group(all P<0.05).Postoperative BCS score was higher and VAS score was lower in the observation group compared with the control group(both P<0.05).Levels of PCT,CRP and WBC increased in both groups after surgery compared with preoperative levels,but the increase amplitude was significantly smaller in the observation group(all P<0.05 or<0.01).The total incidence of ad-verse reactions was 3.45%(2/58)in the observation group,significantly lower than 13.79%(8/58)in the control group(P<0.01).Conclusion The combination of RIRS and controlled negative pressure suction sheath contributes to a higher stone-free rate,shorter operative and hospital stay,less intraoperative blood loss and lower medical costs.Meanwhile,it alleviates inflam-matory response and postoperative pain,reduces the incidence of complications,demonstrating favorable safety and feasibility.关键词
