Application of multidisciplinary team model in chronic wound care in orthopedics:A retrospective cohort study
摘要
Abstract
Objective To explore the application effect of the multidisciplinary collaboration(MDT)model in the chronic wound care of orthopedics, and to clarify its influence on wound healing time, infection recurrence rate, patient pain degree, and medical costs,in order to provide evidence for the new model of chronic wound care in orthopedics. Methods A retrospective cohort study was conducted.A total of 160 patients with chronic wounds admitted to the Department of Orthopedics, the 920th Hospital of Joint Logistics Support Force between January 2023 and January 2025 were enrolled and categorized into an MDT group(n=80)and a conventional care group(n=80)based on the protocol received. The conventional care group received routine orthopedic chronic wound care, while the MDT group received a closed-loop nursing management approach consisting of"precise assessment-individualized intervention-dynamic feedback."The wound healing time, infection recurrence rate, number of dressing changes, medical expenses, wound exudate pH,interleukin-6(IL-6)level,Numerical Rating Scale(NRS)pain score, medical cost, and nursing satisfaction were compared between the two groups. Results The MDT group had significantly shorter wound healing time, lower infection recurrence rate, fewer dressing changes, and lower medical costs than the conventional care group(P<0.05).Before intervention, there were no significant differences in wound exudate pH and IL-6 level between the two groups(P>0.05).After intervention, wound exudate pH and IL-6 levels in both groups were significantly lower than those before intervention, with significantly lower values in the MDT group than in the conventional care group(P<0.05).After intervention, the NRS score in the MDT group was(2.1±0.8)points,which was lower than that in the conventional care group(3.5±1.2)points;(t=5.305,P<0.05).The overall patients satisfaction rate in the MDT group(96.25%)was higher than that in the conventional care group(88.75%;χ2=10.320,P<0.05).Conclusions The new nursing model based on MDT achieves precise management of chronic wounds in orthopedics through the integration of multidisciplinary resources, significantly shortening the healing time, reducing infection risk, lowering wound exudate pH,IL-6 levels, and alleviating pain, while also offering cost-effectiveness advantages and high patient acceptance. It represents an optimized approach to orthopedic chronic wound care worthy of clinical promotion.关键词
