Abstract
Objective To compare the effects of different patellar heights on early clinical outcomes and pain in patients undergoing primary total knee arthroplasty(TKA)with patellar resurfacing.Methods A retrospective analysis was conducted on 110 patients who underwent primary unilateral TKA at the Department of Orthopedics,Nanyang Central Hospital from February 2023 to January 2025.According to the preoperative Insall-Salvati(IS)ratio,patients were divided into a normal patellar height group(58 cases)and a patella alta group(52 cases).The postoperative IS ratio,Knee Society Score(KSS),Oxford Knee Score(OKS),range of motion(ROM)of the knee joint,and Numerical Rating Scale(NRS)score were compared between the two groups.Additionally,the incidence of postoperative complications in both groups was recorded.Results Preoperatively,the IS ratio in the patella alta group was significantly higher than that in the normal patellar height group(P<0.001).At 6 months postoperatively,the IS ratio in the patella alta group was lower than the preoperative value(P<0.05);but there was no statistically significant difference in the IS ratio between the two groups(P>0.05).At 6 months postoperatively,the KSS score in the normal patella height group[(79.78±8.72)points]was higher than that in the patella alta group[(74.20±8.16)points;t=3.454,P<0.01],while the OKS score in the normal patella height group[(14.13±4.09)points]was lower than that in the patella alta group[(16.65±4.53)points;t=3.066,P<0.01].At 6 months postoperatively,the knee joint ROM in the normal patellar height group was(123.97±5.12)°,which was significantly higher than that in the high patellar height group(121.03±2.54)°,P<0.001.There was no statistically significant difference in the total incidence of postoperative complications between the two groups(P>0.05).Conclusions In primary TKA with patellar resurfacing,patients with patella alta can achieve effective anatomical correction at 6 months postoperatively with a complication risk comparable to that of the normal patellar height group,however,functional recovery and pain control are inferior to those in the normal patellar height group.关键词
