鳖甲灸推熨法联合仿生物电刺激疗法治疗产后肾阳虚型压力性尿失禁患者的疗效观察

Abstract
Objective To observe the effects of the combined therapy of massage and fomentation method by soft-shelled turtle shell moxibustion and biomimetic electrical stimulation on pelvic floor muscle function and quality of life in postpartum patients with stress urinary incontinence(SUI)of kidney yang deficiency syndrome.Methods A total of 96 postpartum patients with SUI admitted to Zhangjiagang Hospital of Traditional Chinese Medicine from July 2024 to December 2024 were selected as study subjects and randomly divided into a control group and an observation group using a random number table method,with 48 cases in each group.Both groups received conventional Western medical treatment.On this basis,the control group was treated with biomimetic electrical stimulation therapy,while the observation group received biomimetic electrical stimulation therapy combined with the massage and fomentation method by soft-shelled turtle shell moxibustion.The therapeutic outcomes,1-hour urine leakage(detected by the 1-hour urine pad test),the impact of urinary incontinence on daily life[assessed by the International Consultation on Incontinence Questionnaire-Simple Form(ICI-Q-SF)],urinary incontinence-related quality of life[assessed by the Incontinence Impact Questionnaire-Quality of Life(I-QOL)],pelvic floor muscle strength(assessed by the Oxford grading scale),and pelvic floor muscle surface electromyographic values(evaluated using the MyoTrac series of bio-stimulation feedback devices)were compared between the two groups.Results The total effective rate of the observation group was 95.83%(46/48),which was significantly higher than that of the control group(77.08%,37/48)(P<0.05).At 4 weeks and 6 months post-treatment,the 1-hour urinary leakage volume in both groups was significantly lower than before treatment(P<0.05).Additionally,the observation group exhibited significantly lower 1-hour urinary leakage volume than the control group(P<0.05).At 4 weeks and 6 months post-treatment,the ICI-Q-SF scores in both groups were significantly lower than those before treatment(P<0.05).Moreover,the observation group showed significantly lower ICI-Q-SF scores than the control group(P<0.05).At 4 weeks and 6 months post-treatment,the I-QOL scores in both groups were significantly higher than those before treatment(P<0.05).Additionally,the observation group demonstrated significantly higher I-QOL scores than the control group(P<0.05).At 4 weeks and 6 months post-treatment,the MOS scores in both groups were significantly higher than those before treatment(P<0.05).Furthermore,the observation group exhibited significantly higher MOS scores than the control group(P<0.05).At 4 weeks and 6 months post-treatment,the maximum values of rapid contraction,mean values of sustained contraction,and mean values of endurance contraction in both groups were significantly higher than those before treatment(P<0.05).Moreover,the observation group showed significantly higher values for the above parameters than the control group(P<0.05).Conclusion Compared with postpartum patients with SUI of kidney yang deficiency syndrome treated solely with biomimetic electrical stimulation therapy,those receiving combined therapy-biomimetic electrical stimulation plus the massage and fomentation method by soft-shelled turtle shell moxibustion based on the theory of"treating qi prior to treating fluid"exhibited reduced urine leakage after treatment,which was beneficial for reducing the impact of urinary incontinence on daily life,improving pelvic floor muscle strength and pelvic floor muscle surface electromyographic values,promoting the recovery of pelvic floor function,and reducing the impact of urinary incontinence on quality of life.关键词
