针刀联合毫针治疗腰椎间盘突出症寒湿凝滞型临床研究
Clinical study on acupotomy combined with filiform needle acupuncture in treating lumbar disc herniation of cold-dampness stagnation type

Abstract
Objective:To observe the effects of acupotomy combined with filiform needle acupuncture on the Oswestry Disability Index(ODI)score,Visual Analog Scale(VAS)score,and EuroQol Five-Dimension(EQ-5D)score in patients with lumbar disc herniation(LDH)of cold-dampness stagnation type.Methods:A total of 72 LDH patients admitted to the Orthopedics Department of the Affiliated Hospital of Tianjin Academy of Traditional Chinese Medicine from May 2024 to December 2024 were selected and randomly divided into an experimental group and a control group using a random number table method,with 36 cases in each group.Patients in the control group received acupotomy treatment.After the acupotomy tool reached the lesion site,a"cross"incision was made 2-3 times until adequate release was achieved,addressing adhe-sions and scars at the transverse process area,the ligamentum flavum,and the supraspinous and interspinous ligaments from L3 to L5.Each treatment point was manipulated for approximately 30 seconds,once a week,with 4 weeks constituting one course.Patients in the experimental group received acupotomy combined with filiform needle acupuncture.The filiform needle acupuncture included both body acupuncture and scalp acupuncture.Body acupuncture acupoints:Jiaji(EX-B2),Shenshu(BL23),Dachangshu(BL25),Yaoyangguan(GV3),Guanyuanshu(BL26),Zhibian(BL54),Huantiao(GB30),Yanglingquan(GB34),Weizhong(BL40),Chengshan(BL57),Kunlun(BL60),Xuanzhong(GB39),and Ashi acupoints.Scalp acupuncture acupoints:middle parietal line or the upper 1/5 of the contralateral parietotemporal posterior oblique line.After achieving deqi,the needles were retained for 30 minutes,with manipulation performed every 10 minutes.Treat-ment was given once daily,five times per week,with 4 weeks constituting one course.The ODI,VAS and EQ-5D scores and total effective rate were compared between the two groups before and after treatment.Results:Compared with the pre-treatment status,both groups showed significant improvements in ODI,VAS and EQ-5D scores after 4 weeks of treatment,and the experimental group was superior to the control group,with statistically significant differences(P<0.05,P<0.01).The clinical total effective rate in the experimental group(94.4%,34/36)was higher than that in the control group(88.9%,32/36),and the difference was statistically significant(P<0.05).Conclusion:Both acupotomy alone and acupotomy combined with filiform needle acupuncture can effectively improve the ODI,VAS and EQ-5D scores in patients with LDH of cold-dampness stagnation type.However,acupotomy combined with syndrome-based filiform needle acupuncture yields better therapeutic effects.关键词
