飞针浅刺治疗脊髓损伤后尿潴留的疗效观察
Observation on the efficacy of shallow flying needle insertion for urinary retention after spinal cord injury

Abstract
Objective To observe the clinical efficacy of shallow flying needle insertion for urinary retention after spinal cord injury.Method Ninety patients with urinary retention after spinal cord injury were randomized to a conventional group(30 cases),a deep needling group(30 cases)and a shallow needling group(30 cases).All the three groups received basic treatments including intermittent clean catheterization and fluid intake guidance.The conventional group received conventional acupuncture;the deep needling group,Lingnan flying acupuncture manipulation with needles inserted deeply into the muscle layer of the abdominal acupuncture zone;the shallow needling group,Lingnan flying acupuncture manipulation with needles inserted shallowly into the subcutaneous fat layer of the abdominal acupuncture zone.Urodynamic indicators,urinary voiding condition and the quality-of-life scale score were observed in the three groups before and after treatment.The clinical efficacy and the incidence of adverse reactions were compared between the three groups.Result After treatment,residual urine volume decreased(P＜0.05)and maximum urine flow rate,maximum detrusor pressure and maximum bladder capacity increased(P＜0.05)in the three groups compared with before.After treatment,residual urine volume was lower in the shallow needling group than in the conventional group(P＜0.05);maximum urine flow rate,maximum detrusor pressure and average urine volume were higher(P＜0.05)and urinary voiding frequency was lower(P＜0.05)in the shallow needling group than in the deep needling and conventional groups;the quality-of-life scale score was lower in the shallow needling group than in the conventional group(P＜0.05).Maximum urine flow rate and average urine volume were higher in the deep needling group than in the conventional group(P＜0.05).After treatment,maximum bladder capacity had no statistically significant differences between the three groups(P＞0.05).The total efficacy rates of traditional Chinese medicine and Western medicine had statistically significant differences between the three groups(P＜0.05).The total efficacy rate of traditional Chinese medicine was higher in the shallow needling and deep needling groups than in the conventional group(P＜0.05).The total efficacy rate of Western medicine was higher in the shallow needling group than in the conventional group(P＜0.05).No obvious adverse reaction occurred in the three groups.Conclusion Shallow insertion of flying needles can effectively improve urinary voiding function and the quality of life in patients with urinary retention after spinal cord injury.Its therapeutic effect is better than that of conventional acupuncture.关键词
