Exploring Role of Mitophagy and Macrophage Polarization in Diabetic Kidney Disease Based on Theory of"Spleen Failing to Disperse Essence and Internal Accumulation of Turbid Toxin"

Abstract
Diabetic kidney disease(DKD)is a microvascular complication caused by diabetes and is one of the main causes of end-stage renal disease.Although its pathogenesis has not been fully elucidated,chronic microinflammation is considered an important factor contributing to the aggravation of renal injury in DDK patients.Mitochondrial autophagy and macrophage polarization are the key to maintaining cellular homeostasis and inflammatory balance.Studies have shown that abnormal mitochondrial autophagy can induce an imbalance in macrophage polarization,leading to the accumu-lation of inflammatory factors in the glomeruli and renal interstitium,causing renal pathological damage and accelerating the progression of DKD.Therefore,restoring the balance of mitochondrial autophagy and macrophage polarization is an im-portant way to improve the chronic microinflammation state in DKD.Traditional Chinese medicine believes that"spleen failing to disperse essence and internal accumulation of turbid toxins"is the basic pathogenesis of DKD.The normal func-tion of mitochondria is related to the function of spleen to disperse essence,and mitochondrial dysfunction can induce macrophage polarization imbalance,resulting in an inflammatory response,which is equivalent to the internal accumulation of turbid toxins in traditional Chinese medicine.The two work together to affect the occurrence and development of DKD.Therefore,based on the theory of"spleen failing to disperse essence and internal accumulation of turbid toxins",this arti-cle explored the relationship among mitochondrial autophagy,macrophage polarization and DKD from different perspec-tives,as well as the mechanism of traditional Chinese medicine in regulating mitochondrial autophagy and macrophage po-larization in the treatment of DKD,in order to provide new therapeutic ideas for the clinical treatment of DKD.关键词
