Healthcare-Associated Infections in a General Teaching Hospital in Hubei Province,2022-2024:A Cross-Sec-tional Survey

Abstract
Objective To investigate the epidemiological status of healthcare-associated infections(HAIs)in a general teaching hospital in Hubei Province and analyze the influencing factors,to provide data support and evidence for effective HAI prevention and control.Methods A cross-sectional survey was conducted on August 31 of each year from 2022 to 2024(00:00-24:00)among all hospitalized patients.Medical record reviews were combined with bedside clinical examinations in this study to analyze the trends in HAI prevalence,and indicators included HAI prevalence,involved departments,caus-ative pathogens,multidrug-resistant organism(MDRO)detection,and pathogen detection rate before antimicrobial thera-py.Results A total of 6955 hospitalized patients were surveyed across the three years.The HAI prevalence rates were 1.26%,0.95%,and 0.95%,respectively,with no significant difference over the study period(P＞0.05).The overall aver-age HAI prevalence rate was 1.05%,and the average initial infection rate was 1.24%.The ICU had the highest HAI preva-lence rate(26.15%),followed by the departments of Neurosurgery,Hematology,Orthopedics,and Neurology.Lower re-spiratory tract was the predominant HAI site(43.02%),followed by bloodstream and urinary tract infections.A total of 78 pathogenic strains were isolated from HAI patients,and 127 MDROs were identified under hospital surveillance,yielding a detection rate of 23.26%.The average antimicrobial usage rate was 34.10%.The pathogen detection rates before antimicro-bial therapy were 82.06%,85.89%,and 89.5%over the three years,respectively,showing an upward trend.Conclusion The HAI prevalence of this hospital is consistent with the national average and remains within a reasonable control range.However,HAIs have distinct clustering in specific departments or sites,with the ICU and Neurosurgery as vulnerable de-partments requiring strengthened infection prevention and control.The hospital should promote rational antimicrobial use,reduce MDRO detection,and reinforce infection control measures in high-risk departments and sites.关键词
