黄连温胆汤合酸枣仁汤加减联合决奈达隆治疗痰火扰心型阵发性心房颤动临床研究
Clinical Study on Modified Huanglian Wendan Decoction plus Suanzaoren Decoction Combined with Dronedarone for Paroxysmal Atrial Fibrillation of Phlegm-Fire Disturbing Heart Type

Abstract
Objective:To observe the clinical efficacy of modified Huanglian Wendan Decoction plus Suanzaoren Decoction combined with Dronedarone in the treatment of paroxysmal atrial fibrillation(PAF)of phlegm-fire disturbing heart type.Methods:A retrospective analysis was conducted on 109 patients with PAF of phlegm-fire disturbing heart type admitted to Guangdong Provincial Hospital of Chinese Medicine from July 2020 to July 2025.After excluding 9 cases based on exclusion criteria,100 patients were finally enrolled.They were divided into the western medicine group(48 cases)and the integrated traditional Chinese and western medicine group(short as the integrated group,52 cases)according to the treatment regimens.The western medicine group received Dronedarone alone,while the integrated group received modified Huanglian Wendan Decoction plus Suanzaoren Decoction combined with Dronedarone.The clinical efficacy,disease recovery,echocardiographic parameters[left ventricular ejection fraction(EF),left ventricular end-systolic diameter(LVESD),left atrial diameter(LAD)],serum cardiac function indicators[lactate dehydrogenase(LDH),N-terminal pro-brain natriuretic peptide(NT-proBNP)],and the incidence of adverse reactions were compared between the two groups.Results:After treatment,the total effective rate was 96.15%(50/52)in the integrated group and 81.25%(39/48)in the western medicine group,and the difference between the two groups was statistically significant(P＜0.05).After treatment,the 24-hour average ventricular rate,frequency of atrial fibrillation,and duration of atrial fibrillation attacks decreased in both groups compared with those before treatment(P＜0.05),and in the integrated group,the 24-hour average ventricular rate was lower,the frequency of atrial fibrillation was less,and the duration of atrial fibrillation attacks was shorter than those in the western medicine group(P＜0.05).After treatment,EF levels increased in both groups compared with those before treatment(P＜0.05),while LVESD and LAD decreased compared with those before treatment(P＜0.05);the integrated group showed a higher EF level(P＜0.05)and smaller LVESD and LAD(P＜0.05)than the western medicine group.After treatment,LDH and NT-proBNP levels decreased in both groups compared with those before treatment(P＜0.05),and the integrated group had lower levels of LDH and NT-proBNP than the western medicine group(P＜0.05).During treatment,the incidence of adverse reactions was 17.31%(9/52)in the integrated group and 16.67%(8/48)in the western medicine group,with no statistically significant difference between the two groups(P＞0.05).Conclusion:Modified Huanglian Wendan Decoction plus Suanzaoren Decoction combined with Dronedarone can improve the therapeutic efficacy,enhance cardiac function,and promote disease recovery in patients with PAF of phlegm-fire disturbing heart type.关键词
