A Questionnaire Survey on Blood Management Practices During Pediatric Cardiopulmonary Bypass in the North China Region
摘要
Abstract
Objective To investigate current clinical practices in blood management during pediatric cardiopulmonary bypass(CPB)across cardiac surgery centers in the North China Region, and to identify gaps and areas for improvement in patient blood conservation. Methods A questionnaire survey was conducted among pediatric CPB practitioners from 15 cen-ters in the North China Region. The survey addressed 16 common blood management measures during pediatric CPB,as well as key aspects including CPB transfusion thresholds, heparin priming dosage,protamine-to-heparin reversal ratio, and whether intraoperative blood product requests involved multidisciplinary shared decision-making.Results The majority of centers(>60%)routinely or selectively adopted core patient blood management practices, including the use of oxygenators with in-tegrated arterial filters(93.3%),implementation of structured blood management protocols(86.6%),vacuum-assisted venous drainage(80%),and individualized tubing packs(73.3%).However,considerable variations were observed in transfusion thresholds, heparin priming doses, and protamine reversal strategies. Moreover, differences in the protamine-to-heparin reversal ratio were closely linked to whether the heparin within the CPB circuit was included in the dose calculation. Conclusion This study reveals substantial consensus on pediatric blood management concepts among most centers in the North China Region.Nevertheless, certain management strategies require refinement, and further standardization of key pro-cedural steps is warranted.关键词
