麻精药品信息化闭环管理体系的构建与实践
Construction and Practice of an Information-Based Closed-Loop Management System for Narcotic and Psychotropic Drugs

Abstract
Objective To construct an information-based closed-loop management system for narcotic and psychotropic drugs.Methods Based on the hospital information system(HIS),a"Three-phase,Four-module"progressive strategy was adopted to sequentially advance the construction of four collaborative closed-loop management modules.In phase Ⅰ(from April 2022 to June 2023),module 1(internal closed-loop management within the pharmacy department)and module 2(outpatient pharmacy-clinic collaborative closed-loop management)were established,achieving dual closed-loop systems between multiple teams within the pharmacy department and between outpatient pharmacies and outpatient clinics.In phase Ⅱ(from July 2023 to June 2024),module 3(anesthesia department pharmacy-anesthesia collaborative closed-loop management)was established,achieving closed-loop system between inpatient pharmacy and the anesthesia department.In phase Ⅲ(from July 2024 to June 2025),module 4(inpatient physician-pharmacist-nurse collaborative closed-loop management)was established,achieving closed-loop system between inpatient pharmacy and inpatient departments.Results After the hospital established the information-based closed-loop management system for narcotic and psychotropic drugs,dedicated medical records,prescriptions,special account books,and special registrations were fully electronic.As of December 2024,electronic dedicated medical records had been established for 525 long-term patients,with outpatient electronic dedicated medical record coverage exceeding 98%and outpatient pain assessment completion rate reaching 100%.The entire process of narcotic and psychotropic drug management in hospital was fully traceable,achieving a 100%inventory accuracy rate,and the outpatient consultation process was reduced from 7 steps to 5 steps.In 2024,the outpatient prescription review pass rate exceeded 98%,compared with 95%before system implementation,an increase of 3 percentage points,with no prescription format defects.After module 2 established,outpatient narcotic and psychotropic drug prescriptions achieved 100%full-process prescription review coverage.In 2024,a total of 166 419 electronic prescription registrations were completed,saving approximately 1 100 working days(equivalent to 4 full-time equivalent positions),and the inpatient pharmacy and anesthesia department reduced dedicated positions by 1 and 0.5,respectively.In 2023,the system identified 16 high-risk patients,whose risky behaviors were controlled after intervention.Among 171 patients served at the pain pharmacy clinic,17.54%(30/171)were classified as high-risk,and no red-alert patients were reported on the provincial platform in 2024.The total project investment was CNY 81 760,with short-term cost recovery expected,and long-term improvements in hospital management efficiency and service quality,yielding significant social and economic benefits.Conclusion The full-process collaborative closed-loop management system for narcotic and psychotropic drugs based on the HIS operates stably,significantly improving the management efficiency and safety of narcotic and psychotropic drugs,and provides a low-cost,replicable solution for small and medium-sized medical institutions.关键词
