韦氏三联九针联合益气明目汤治疗高原地区非增殖性糖尿病视网膜病变的疗效观察
Observation on the Therapeutic Efficacy of WEI's Triple NineNeedles Technique Combined with Yiqi Mingmu Decoc-tion in Treating Non-proliferative Diabetic Retinopathy in High-altitude Regions

Abstract
OBJECTIVE To observe the clinical efficacy of WEI's Triple Nine Needles combined with Chinese medicines in the treatment of non-proliferative diabetic retinopathy(NPDR)in the plateau area.METHODS A total of 120 NPDR patients(120 eyes)diagnosed and treated in Qinghai Provincial Hospital of Traditional Chinese Medicine(TCM)from September 2023 to September 2024,who were differentiated as Qi-Yin deficiency syndrome in TCM,were enrolled in this study.The patients were randomly divided into four groups,with the right eye included in the study for each patient:Western medicine group(Group A),acupuncture group(Group B),TCM group(Group C),and acupuncture combined with TCM group(Group D).Finally,27 cases(27 eyes)were included in Group A,24 cases(24 eyes)in Group B,30 cases(30 eyes)in Group C,and 23 cases(23 eyes)in Group D.Group A was treated with oral calcium dobesilate capsules alone.On the basis of Group A's treatment,Group B was supplemented with WEI's Triple Nine Needles,Group C with oral Yiqi Mingmu Decoction,and Group D with the combination of WEI's Triple Nine Needles and oral Yiqi Mingmu Decoction.All four groups received continuous treatment for 12 weeks.Before and after treatment,the following indices were detected:Best corrected visual acuity(BCVA),mean visual field sensitivity(MS),central retinal thickness(CRT)of the macula,fundus symptom score,total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),TCM syndrome score,and intraocular pressure(IOP).RESULTS(1)Ophthalmic indicators:Comparison before and after treatment showed that BCVA improved,MS increased,and CRT decreased in all four groups after treatment compared to before treatment,the fundus symptom scores in groups B,C and D decreased after treatment compared to before treatment,the differences were all statistically significant(P<0.05).After treatment,comparisons among the four groups showed statistically significant differences in BCVA,MS,and fundus symptom scores(P<0.05).The BCVA,MS,and fundus symptom scores in group D were superior to those in groups A,B,and C,the fundus symptom scores in groups B,C,and D were lower than those in group A,the differences were all statistically significant(P<0.05).After treatment,comparison of CRT among the four groups showed no statistically significant difference(P>0.05).(2)Serum indicators:Comparison before and after treatment showed that TC,LDL-C,TNF-α,and IL-6 decreased in all four groups after treatment compared to before treatment,the differences were all statistically significant(all P<0.05).After treatment,comparisons among the four groups showed statistically significant differences in TC,LDL-C,TNF-α,and IL-6(P<0.05).After treatment,TC,IL-6,and TNF-α in group D were lower than those in groups A,B and C,and LDL-C in group D was lower than that in groups A and B,the differences were all statistically significant(all P<0.05).(3)TCM syndrome scores:Comparison before and after treatment showed that the TCM syndrome scores in groups B,C,and D decreased after treatment compared to before treatment,the differences were all statistically significant(all P<0.05).After treatment,the comparison among the four groups showed a statistically significant difference(P<0.05),the TCM syndrome scores in groups B,C,and D were lower than those in group A,those in groups C and D were lower than those in group B,that in group D was lower than those in group C,the differences were all statistically significant(all P<0.05).(4)Adverse reactions:Comparison of the incidence of adverse reactions among the four groups showed no statistically significant difference(P>0.05).CONCLUSIONS WEI's Triple Nine Needles combined with Yiqi Mingmu Decoction for treating NPDR with Qi-Yin deficiency syndrome in plateau areas can effectively improve patients'BCVA and retinal light sensitivity,reduce macular edema,meliorate fundus damage in NPDR patients with Qi-Yin deficiency syndrome in the plateau area.Its clinical efficacy is superior to that of single Western medicine,single acupuncture,or single TCM treatment.关键词
