Treatment of Diabetic Macular Edema Based on the Idea of"Unblocking Yang Does Not Depend on Warming but Promoting Urination"
摘要
Abstract
Diabetic macular edema(DME)is a common complication of diabetic retinopathy and a major cause of visual impairment,with blurred and distorted vision as the main clinical manifestations.In Traditional Chinese Medicine(TCM),it falls into the category of dim vision.Its pathogenesis is characterized by simultaneous consumption of Qi and Yin.With prolonged illness,Yin impairment involves Yang,leading to deficiency of both Yin and Yang,blockage of eye collaterals,intermingled phlegm and blood stasis,and injury to the eye system.Based on the theoretical idea of"Unblocking Yang does not depend on warming but promoting urination"put forward by YE Tianshi,a distinguished warm-heat school physician in the Qing Dynasty,the author holds that the core pathogenesis of some DME patients lies in Yang stagnation and internal retention of water-dampness.Therapeutic methods include promoting diuresis and draining dampness,activating blood circulation to dissipate blood stasis,resolving phlegm and dissipating masses to unblock Yang and disperse stagnation,combined with tonifying and ascending Qi,invigorating spleen and harmonizing the middle energizer to strengthen healthy Qi for consolidation.From the theory of"Unblocking Yang does not depend on warming but promoting urination",this paper discusses the etiology,pathogenesis,therapeutic principles and methods of DME,so as to provide new ideas and approaches for its TCM treatment.关键词
