橡皮条牵引技术在旋股外侧动脉穿支皮瓣血管蒂切取中的应用
The application of rubber band traction technique in the dissection of vascular pedicle for lateral circumflex femoral artery perforator flap

Abstract
Objective To evaluate the safety and efficacy of the rubber band traction technique in dissecting the vascular pedicle of the lateral circumflex femoral artery perforator flap.Methods This retrospective study enrolled 110 patients(84 male,26 female;median age 44.7 years,range 4-85 years)between July 2021 and June 2024,involving a total of 223 perforators.All vascular pedicles were harvested using a customized rubber band traction technique.Perforators were preoperatively localized via CTA and high-frequency ultrasound.Pedicle dis-section time,vascular injury,and flap survival were recorded.Results The mean dissection time per perforator pedicle was(9.3±2.3)minutes.No main trunk transection occurred intraoperatively.Postoperative vascular crisis occurred in 4 cases(3 venous,1 arterial),with 3 flaps surviving after emergency exploration and 1 fail-ing.Donor-site complications included 2 wound infections,1 partial dehiscence,and 1 partial skin graft necro-sis;all resolved with dressing changes.During a mean follow-up of(24.3±7.2)months,muscle strength and knee joint mobility at donor sites remained normal.Conclusion The rubber band traction technique offers advan-tages of minimal trauma,time efficiency,ease of mastery,and material accessibility.It is a reliable method for dissecting the vascular pedicle of lateral circumflex femoral artery perforator flaps and warrants clinical adoption.关键词
