Exploration on"Holistic Regulation of Body and Spirit"in the Treatment of Immune Checkpoint Inhibitor-Related Cutaneous Adverse Events from the Perspective of"Tranquility Ensures Closure of the Striae and Interstices"
摘要
Abstract
Drawing upon the theory"tranquility ensures closure of the striae and interstices"in the Huangdi Neijing,this paper explores the application of"holistic regulation of body and spirit"in the treatment of cutaneous immune-related adverse events(cirAEs).Mental tranquility constitutes the intrinsic core for maintaining the integrity of striated layer and resisting external pathogens,whereas internal cancer toxin disturbance and the warm medicinal properties of immune checkpoint inhibitors(ICIs)are external factors that disrupt this state of tranquility.The vicious cycle of"accumulation of pathogenic toxin(physical damage)-insecurity of the striated layer-destabilization of heart and spirit(spiritual impairment)"constitutes a key pathogenesis in the occurrence and aggravation of cirAEs.Accordingly,therapeutic strategies for cirAEs should not be limited to the management of cutaneous manifestations alone,but should place particular emphasis on regulating the heart and spirit.By"harmonizing body and spirit to reestablish tranquility",this approach aims to interrupt the reciprocal deterioration of physical and spiritual domains.On this basis,a treatment strategy centers on"stage-based detoxification with consolidation of the striae and interstices as the key",thereby achieving the therapeutic state described as"tranquility ensures closure of the striae and interstices".Clinical practice has shown that syndrome differentiation and treatment guided by this theory yield favorable outcomes.The relevant theory and clinical experience are summarized to provide insights for the management of cirAEs.关键词
