中高位直肠癌的手术现状及研究进展
Current status and research progress of surgery for middle and high rectal cancer
摘要
Abstract
Rectal cancer is a common malignant tumor of the digestive tract,and its diagnosis and treatment strategies are highly dependent on the accurate localization of the tumor.Based on rectal anatomical segmentation principles,rectal cancer is clinically classified into ultra-low,low,middle,and high subtypes.Tumors at different segments show significant differences in surgical approaches,the scope of lymph node dissection,neurological function protection,and the indications for neoadjuvant therapy.For a long time,low rectal cancer,espe-cially ultra-low rectal cancer involving the anal sphincter as well as sphincter preservation,has been the focus of clinical research and con troversy.However,with the popularization of the principle of total mesorectal excision(TME),the innovation of minimally invasive techniques,and the emergence of the total neoadjuvant therapy model,the concept of diagnosis and treatment for middle and high rectal cancer is con stantly updated and optimized.This article aims to systematically elaborate on the principles and specific methods of rectal cancer classifica-tion,focus on the current status of surgical treatment for middle and high rectal cancer,and deeply explore the research progress in cutting-edge fields such as minimally invasive surgery,functional protection,individualized treatment,and combined organ resection,so as to provide a reference for clinical practice.关键词
