Abstract
The incidence of chronic digestive system diseases continues to rise, and conventional in-hospital nursing struggles to meet the demands for long-term,continuous,and individualized management."Internet+home nursing"leverages digital technologies to pro-vide extended care beyond the hospital setting, while the knowledge-attitude-practice(KAP)education model drives health behavior change through three stages:knowledge acquisition, attitude formation, and practice adoption. The integration of these two approaches can precisely address the core issue of patients"knowing but not believing, and believing but not practicing".It improves disease awareness, treatment be-liefs,and self-management behaviors, thereby reducing the risk of disease recurrence and readmission, and enhancing quality of life. This pa-per systematically reviews the theoretical framework of this integrated model, with a focus on evaluating its clinical efficacy in patients with inflammatory bowel disease, liver cirrhosis, and gastrointestinal tumors,as well as those with indwelling tubes and stomas, and gastrointestinal tumors.Furthermore,it proposes actinable regionalized implementation strategies to address current challenges such as the digital divide,un-even resource distribution, and inadequate quality control. This review aims to provide an evidence-based reference for gastroenterology spe-cialist nurses in delivering precise and continuous home-based chronic disease management.关键词
