Abstract
The aging process of China's population has led to a rapid increase in the demand for rehabilitation services among the elderly.Taking the Department of Rehabilitation Medicine at the Wenhui Street Community Health Service Center in Gongshu District,Hangzhou,as a practical case,this paper examines the current state of elderly rehabilitation services in the area,analyzes core challenges in resource allocation,service capacity,ustainable development,and explores optimization pathways from four dimensions:service network,smart technology,talent development,and policy support.The proposed strategies include establishing a"1+N"collaborative service network with the center as the hub,standardizing division of labor and referral procedures,applying smart rehabilitation systems for remote monitoring and home-based guidance,promoting a"three-tier"rehabilitation talent training system,and advocating for the inclusion of core elderly rehabilitation services in medical insurance and basic public health service packages.Practice has shown that innovative service models and strengthened support systems are key to improving the accessibility and effectiveness of primary community rehabilitation services.关键词
