TROP2表达在HER2阳性胃癌中的预后价值及ADC联合治疗潜力探索
Prognostic value of TROP2 expression and potential for antibody-drug conjugate combination therapy in HER2-positive gastric cancer

Abstract
Objective To investigate the expression of human trophoblast cell-surface antigen 2(TROP2)in human epidermal growth factor receptor 2(HER2)-positive gastric cancer,analyze its relationship with clinicopatho-logical features and prognosis,and explore the therapeutic potential of antibody-drug conjugate(ADC)targeting TROP2 in this population.Methods TROP2 expression was evaluated by the EnVision two-step immunohistochemi-cal method in tumor tissues from 63 patients with HER2-positive gastric cancer.Associations of TROP2 expression with clinicopathological features and overall survival(OS)were analyzed using the Kaplan-Meier method and Cox re-gression.Results The rate of high TROP2 expression was significantly higher in HER2-positive gastric cancer than in adjacent normal gastric tissue[66.7%(42/63)vs 23.1%(12/52),P＜0.001].High TROP2 expression was signifi-cantly associated with poor differentiation,greater depth of invasion,lymph node metastasis,lymphovascular inva-sion,advanced pTNM stage,and a high Ki67 proliferation index(all P＜0.05).OS was significantly associated with TROP2 expression,tumor differentiation,depth of invasion,pTNM stage,and lymphovascular invasion(all P＜0.05).Univariate Cox regression showed that TROP2 expression,tumor differentiation,pTNM stage,vascular inva-sion and deep infiltration were associated with OS(all P＜0.05).Multivariate Cox regression further identified TROP2 expression as an independent prognostic factor in HER2-positive gastric cancer(HR=2.784,95%CI:1.174-6.601,P＜0.05).Conclusion TROP2 is associated with tumor progression and is an independent adverse prognostic factor in HER2-positive gastric cancer.Combined assessment of biomarkers such as HER2 and TROP2 may inform treatment decisions and provide a rationale for developing TROP2-targeted ADC-based therapies.关键词
