摘要

Abstract

Objective To design a music training program incorporating sign language elements, and evaluate its feasibility and effectiveness as a non-pharmacological intervention in promoting the brain functions(particularly auditory cognitive functions)and hand functions in older adults care and health services. Methods A prospective study was conducted in inpatients and outpatients from January to December 2025 in the Geriatrics Department of Huadong Hospital Affiliated to Fudan University,a music training rehabilitation nursing team was established.A total of 91 older adults(mean age 82.3±9.4 years)were enrolled and completed an 8-week music training course(1 session per week,1.5 h per session)in four batches. The training content included breathing exercises synchronized with music, rhythm tapping using small instruments such as tambourine sticks, alternating finger manipulation of ear acupoints, and learning sign language songs.Auditory discrimination(frequency, intensity, temporal resolution),auditory attention(Go/No-go test),auditory memory(auditory digit span test),auditory executive function(auditory Stroop test),and finger function(finger-tapping test)were assessed before and after the intervention.Repeated-measures ANOVA and ANCOVA were used for statistical analyses. Results After the music training, their overall auditory perception and discrimination ability significantly improved compared to before the training(P<0.05).Among them, the improvements in frequency and temporal discrimination were particularly significant(P<0.05),while the change in intensity resolution was not statistically significant(P=0.815).The auditory digit memory span and auditory attention significantly improved compared to before the training(P<0.05).Under the condition of verbal sound-meaning interference, their task response speed significantly accelerated(P<0.05).In terms of hand function, the tapping speed of fingers on both hands improved compared to before training.Among them, the differences in the alternating speed between the middle finger and ring finger of the left hand, between the ring finger and little finger,as well as the alternating speed of fingers on the right hand, all had statistical significance(P<0.05).Conclusion Music training incorporating sign language elements is an innovative and accessible healthcare intervention model for the older adults. It can effectively enhance their brain function and hand function to a certain extent, especially showing positive effects in auditory cognitive ability and hand dexterity. This training is both interesting and practical, with potential for promotion. It can provide a non-pharmacological nursing and healthcarepromotion strategy with practical value for the prevention of cognitive and functional decline in the older adults.
