LR-PRP用于2型糖尿病伴慢性牙周炎患者拔牙后的效果
Effects of LR-PRP in Patients with Type 2 Diabetes Mellitus with Chronic Periodontitis after Tooth Extraction

Abstract
Objective:To explore the effects of leukocyte-rich platelet-rich plasma (LR-PRP)in patients with type 2 diabetes mellitus(T2DM)with chronic periodontitis after tooth extraction.Method:A total of 90 patients with T2DM with chronic periodontitis after tooth extraction who visited Ganzhou People's Hospital from May 2024 to May 2025 were selected for the study and randomly divided into the observation group(n=45)and the control group(n=45).The control group received conventional treatment after tooth extraction,while the observation group was given LR-PRP treatment after tooth extraction.The alveolar socket bone mineral density of the two groups before the operation and 12 weeks after the operation were compared.The scores of modified Masse index and VAS of the two groups were compared at 30 minutes and 1,4,12 weeks after the operation.The levels of gingival crevicular fluid cytokine[platelet-derived growth factor AB(PDGF-AB),transforming growth factor-β1(TGF-β1),IL-1β,IL-4,vascular endothelial growth factor(VEGF)]in the two groups before the operation and 4 weeks after the operation were compared.Result:There was no statistically significant difference in the alveolar socket bone mineral density between the two groups before the operation(P>0.05).At 12 weeks after the operation,compared with the control group,the alveolar socket bone mineral density of the observation group was higher(P<0.05).At 30 minutes and 1,4,and 12 weeks after the operation,the modified Masse index scores of the observation group were lower than those of the control group(P<0.05).The main effect differences of the modified Masse index score among groups and over time were statistically significant(P<0.05),and there were interactions between groups and over time(P<0.05).At 4 weeks after the operation,compared with the control group,the levels of PDGF-AB and VEGF in the observation group were higher,while the levels of IL-4,TGF-β1 and IL-1β were lower(P<0.05).At 30 minutes and 1,4,and 12 weeks after the operation,the VAS scores of the observation group were all lower than those of the control group(P<0.05).The main effects differences of VAS scores among groups and over time were statistically significant(P<0.05),and there were interactions between groups and over time(P<0.05).Conclusion:For patients with T2DM with chronic periodontitis,LR-PRP treatment after tooth extraction can promote alveolar socket bone regeneration,accelerate soft tissue healing,improve the cytokine levels of gingival crevicular fluid,and relieve patients'pain.关键词
