Abstract
Objective:To investigate the clinical effects of transfrontal approach neuroendoscopic hematoma evacuation and transtemporal craniotomy under microscopic hematoma evacuation in patients with hypertensive intracerebral hemorrhage(HICH).Method:A total of 100 patients with HICH admitted to Yingtan People's Hospital from March 2022 to March 2025 were retrospectively selected and divided into two groups according to the different surgical methods they adopted. In the control group,46 cases underwent transtemporal craniotomy under microscopic hematoma evacuation,in the observation group,54 cases underwent transfrontal approach neuroendoscopic hematoma evacuation.Perioperative indicators, postoperative hematoma evacuation rate, complications and short-term prognosis were compared between the two groups.Result:Compared with the control group, the observation group had shorter operating time, less intraoperative blood loss, and shorter incision length(P<0.05).At 3 days after operation, compared with the control group, the hematoma evacuation rate in the observation group was higher(P<0.05).There were no significant differences in hematoma evacuation rate at 7 days after operation, complication rate and short-term prognosis between the two groups(P>0.05).Conclusion:Transfrontal approach neuroendoscopic hematoma evacuation for HICH possesses minimally invasive advantages including slighter surgical trauma, shorter operative time, and higher early hematoma evacuation efficiency, without increasing the risk of complications, and yields short-term prognosis comparable to that of transtemporal craniotomy under microscopic hematoma evacuation.关键词
