基于真实世界评价针刺治疗小儿弱视疗效的影响因素及对视网膜血流的影响
Evaluation of Influencing Factors and Retinal Blood Flow in Acupuncture Treatment for Children with Amblyopia Based on Real-world Study

Abstract
OBJECTIVE To evaluate the factors influencing the clinical efficacy of acupuncture in treating children with amblyopia based on real-world data and to explore whether the mechanism of acupuncture therapy is related to the promotion of retinal blood flow.METHODS A total of 39 children(57 eyes)aged four to 12 years diagnosed with amblyopia at the Ophthalmology Clinic of the Affiliated Hospital of Shanxi University of Chinese Medicine from December 2022 to December 2023 were enrolled.All children received optical correction.Children with anisometropic amblyopia received acupuncture based on optical correction,while children with ametropic amblyopia received acupuncture combined with patching based on optical correction.The treatment duration was three months.Data on age,gender,refractive status,diopter,amblyopia severity,amblyopia type,and best-corrected visual acuity(BCVA)were collected before treatment.BCVA was recorded after three months of treatment.Fifteen children were randomly selected for measurements of ocular arterial blood flow parameters[peak systolic velocity(PSV),resistance index(RI),and end-diastolic velocity(EDV)in the central retinal artery(CRA),ophthalmic artery(OA),and posterior ciliary arteries(PCV)]before and after acupuncture,which were then analyzed.According to the therapeutic efficacy criteria for amblyopia,51 eyes were assigned to the effective group and six eyes to the ineffective group,and Univariate and multivariate analyses were performed to investigate the therapeutic efficacy of acupuncture in the treatment of amblyopia.RESULTS(1)BCVA:The BCVA(LogMAR)of the 39 children with amblyopia after treatment was significantly better than before treatment(t=13.037,P=0.000).(2)Correlation analysis between efficacy and influencing factors:Logistic regression analysis showed that the child's age,amblyopia type,amblyopia severity,diopter,refractive status,and whether Jingming(BL1)acupoint was needled were not significant influencing factors for the clinical efficacy of acupuncture(P＞0.05).(3)Ocular arterial blood flow values:After acupuncture,the EDV of the OA,CRA,and PCA and the PSV of the PCA all increased compared with baseline(EDV:ZOA=3.711,ZCRA=4.124,ZPCA=4.112,all P=0.000;PSV:ZPCA=4.108,P=0.000).The PSV of the OA and CRA also increased after acupuncture(tOA=9.287,tCRA=10.503,both P=0.000).All these differences were statistically significant.CONCLUSIONS The significant clinical efficacy of acupuncture in treating children with amblyopia is possibly associated with a notable increase in ocular arterial blood flow velocity and retinal blood supply post-acupuncture.关键词
