波形蛋白在感染性与非感染性急性呼吸窘迫综合征患者中的表达差异及其预后价值
Expression difference of vimentin between infectious and non-infectious acute respiratory distress syndrome and its value for prognosis

Abstract
Objective To investigate the expression difference of vimentin between infectious and non-infectious acute respiratory distress syndrome(ARDS)and its value for prognosis.Methods Clinical data of 178 ARDS patients hospitalized in the ICU of People's Hospital of Ningxia Hui Autonomous Region from June 2024 to June 2025 were retrospectively analyzed.Patients were divided into the infectious ARDS group and the non-infectious ARDS group according to the presence or absence of infection.According to the progno-sis(28-day mortality),patients in each group were further subdivided into the survival group and the death group.General data,clinical and laboratory indicators were collected,and the occurrence of acute kidney injury(AKI)was recorded.Logistic regression analysis was used to identify related influencing factors,and receiver operating characteristic(ROC)curve and area under the curve(AUC)were used to evaluate predictive per-formance.Results The height,predicted body weight(PBW),and vimentin level in the infectious ARDS group were significantly higher than those in the non-infectious ARDS group(P<0.05).Compared with the infectious survival group,the infectious death group had significantly higher levels of driving pressure(DP),procalcitonin(PCT),IL-6,high-sensitivity C-reactive protein(hs-CRP),and vimentin,and significantly lower oxygenation index(OI,P<0.05).Compared with the non-infectious survival group,the non-infectious death group had significantly higher level of DP and significantly lower OI(P<0.05).Logistic regression analysis showed that after adjusting for confounding factors such as height and PBW,vimentin was an independent risk factor for prognosis in patients with infectious ARDS(OR=2.18,95%CI:1.21-3.91,P=0.009).The AUC of the ROC curve for predicting prognosis in patients with infectious ARDS was 0.856(P=0.001),with a cutoff value of 12.68 ng/mL,a sensitivity of 0.826,and a specificity of 0.847.Conclusion Serum vimentin has high predictive value for the prognosis of patients with infectious ARDS.关键词
