MRI引导微波消融治疗早期肝细胞癌的疗效评估
Efficacy assessment of MRI-guided microwave ablation for the treatment of early-stage hepatocellular carcinoma
摘要
Abstract
Objective To investigate the safety and efficacy of MRI-guided microwave ablation(MWA)in the treatment of early-stage hepatocellular carcinoma(HCC).Methods A retrospective analysis was conducted on 47 HCC patients who underwent MRI-guided MWA at Interventional Center for Liver Diseases and Oncology of Beijing Youan Hospital,Capital Medical University, between January and December 2016.Baseline characteristics, intraoperative procedural parameters, and adverse events were collected.Outcomes included complete ablation rate, incidence of major complications,1-,3-,and 8-year overall survival(OS)rates and progression-free survival(PFS).Cox regression analysis was performed to identify factors associated with prognosis. Results A total of 47 patients were recruited for this cohort, consisting of 42 males and 5 females.Age ranged from 37 to 79 years, with a mean age of(56.8±1.4)years.The median maximum tumor diameter was 2.5(1.8,3.4)cm.The complete ablation rate was 100%.All patients presented mild transient elevation of transaminases, and liver function recovered within one week after hepatoprotective treatment. No adverse events of Clavien-Dindo grade Ⅱ or higher occurred. The 1-,3-,and 8-year OS rates were 100.0%,95.1%and 82.4%,respectively,while the median PFS was 25(11,28)months.Conclusion MRI-guided MWA for HCC is a safe and effective minimally invasive treatment with a high complete ablation rate,a low complication rate, favorable long-term overall survival and progression-free survival.关键词
