内镜下经十二指肠乳头引流术在胰腺假性囊肿中的应用
Endoscopic transpapillary drainage in the treatment of pancreatic pseudocysts

Abstract
Objective To explore the surgical efficacy of endoscopic transpapillary drainage(TPD)in the treatment of pancreatic pseudocyst(PPC).Methods A retrospective analysis was conducted on 21 PPC patients who underwent TPD at the Department of Hepatobiliary Surgery,Chenzhou First People's Hospital,from January 2021 to December 2024.All patients had a disease duration exceeding 4 weeks,and imaging examinations indicated intact cyst wall,progressive enlargement of the cyst or magnetic resonance cholangiopancreatography(MRCP)showed communication between the cyst and the main pancreatic duct.Patients underwent TPD alone or combined with percutaneous catheter drainage(PCD).Intraoperative and postoperative indicators were analyzed to evaluate postoperative recovery.Results Among the 21 patients,stents or nasopancreatic catheters were successfully placed in 20 cases,resulting in a technical success rate of 95.24%.One patient with a lesion in the pancreatic tail showed unsatisfactory drainage results and required additional percutaneous external drainage.Three patients developed fever after pancreatic stent placement,which was considered to be due to intracystic infection;symptoms were controlled after conversion to nasopancreatic catheter drainage.The remaining patient with a PPC located in the pancreatic uncinate process failed TPD and subsequently underwent percutaneous external drainage.No cases required conversion to laparoscopic or open surgery,and no complications such as pancreatic duct injury,hemorrhage,or pancreatic fistula occurred.Stents or nasopancreatic catheters were removed 4 weeks postoperatively in the 20 successful cases,and no recurrence was observed during a 1-year follow-up.Conclusion TPD demonstrates significant therapeutic efficacy for PPCs connected with the pancreatic duct,with no evident severe complications or recurrence.For the management of PPCs,comprehensive evaluation of the patient's general condition and relevant diagnostic findings is essential to select the optimal treatment strategy,thereby minimizing the risk of recurrence and complications while improving the quality of life for patients.关键词
