单灯笼塑料胆管支架在ERCP碎石术后胆管引流中的应用研究
Application of single-lantern plastic bile duct stent in biliary drainage following ERCP lithotripsy

Abstract
Objective To evaluate the clinical efficacy of a self-designed single-lantern plastic bile duct stent for biliary drainage following endoscopic retrograde cholangiopancreatography(ERCP)lithotripsy.Methods A single-lantern plastic bile duct stent was designed by our team,and a utility model patent was obtained(ZL202220580594.8)and then produced in collaboration with a medical device manufacturer.This prospective study enrolled 112 patients undergoing ERCP lithotripsy for common bile duct stones at the Cholelithiasis Center of East Hospital Affiliated to Tongji University between January and December 2025.Patients were randomly assigned to either an observational group(n=53),receiving single-lantern plastic bile duct stents,or a control group(n=59),receiving conventional integrated plastic bile duct stents.Following endoscopic sphincterotomy(EST)and endoscopic papillary balloon dilation(EPBD),stone extraction was performed using stone basket and balloon.The primary outcomes measured included difficulty score for stent placement,time required for stent placement,rate of successful stent deployment,and the incidence of complications such as stent detachment,enterobiliary reflux,residual stones in the common bile duct,duodenal papillary hemorrhage,acute cholangitis,and liver dysfunction.Results No statistically significant difference was observed between the two groups in difficulty score for stent placement,time required for stent placement,rate of successful stent deployment,the incidence of stent detachment or acute cholangitis(all P>0.05).The incidences of enterobiliary reflux(22.6%vs 45.8%,χ2=6.576,P=0.010),residual stones in the common bile duct(3.8%vs 25.4%,χ2=10.165,P=0.001),duodenal papillary hemorrhage(1.9%vs 42.4%,χ2=25.674,P<0.001),and liver dysfunction(0 vs 10.2%,χ2=3.866,P=0.049)were significantly lower in the observational group than those in the control group.Conclusion Following ERCP lithotripsy,the use of a self-designed single-lantern plastic bile duct stent results in comparable incidences of stent detachment and acute cholangitis to those of an integrated plastic bile duct stent.However,the self-designed plastic bile duct stent demonstrates superior performance,as evidenced by significantly lower incidences of postoperative enterobiliary reflux and residual stones in the common bile duct,as well as duodenal papillary hemorrhage and liver dysfunction.关键词
