经PTCD途径置入带长回收线全覆膜金属支架治疗胆肠吻合口狭窄的临床观察
Clinical observation of fully covered metallic stent with a long retrieval suture placed via PTCD in the treatment of bilioenteric anastomotic stenosis

Abstract
Fully covered metallic stents(FCMS)are effective devices for the treatment of cholangioenterostomy anastomotic stenosis.However,conventionally deployed FCMS through percutaneous transhepatic cholangiodrainage(PTCD)poses considerable retrieval difficulties.We therefore developed a novel FCMS with a long retrieval suture,which allows direct deployment and retrieval entirely through PTCD.To investigate the application value of this modified FCMS for cholangioenterostomy anastomotic stenosis through PTCD,we retrospectively analyzed the clinical data of 25 patients who underwent placement of the modified FCMS through PTCD for cholangioenterostomy anastomotic stenosis at East Hospital Affiliated to Tongji University from January 2024 to December 2024.The operation time,operation satisfaction score,postoperative complications,FCMS indwelling time and related complications,FCMS retrieval time,retrieval complications,follow-up duration,and stenosis recurrence were summarized.A total of 25 patients were included,including 11 males and 14 females,with a mean age of(55.28±7.35)years.The FCMS with a long retrieval suture was successfully placed via PTCD in all patients.The mean operation time was(5.00±0.71)min,and the operation satisfaction score was(8.96±1.02)points.After stent placement,one patient developed acute cholangitis and another developed acute cholangitis complicated by obstructive jaundice.After 3 days of ineffective conservative treatment,cholangiography confirmed that the symptoms were caused by the FCMS obstructing the adjacent bile duct.After the single-pigtail external drainage tube was withdrawn and re-inserted into the obstructed bile duct through the space between the outer wall of the FCMS and the bile duct wall,both patients recovered uneventfully.The FCMS indwelling time was(7.16±0.80)months,during which no complications such as cholangitis,obstructive jaundice,or stent migration occurred.The FCMS retrieval time was(29.96±3.61)s,and no post-procedural complications such as bleeding or cholangitis were observed.No stenosis recurrence was found during the follow-up.Placement of an FCMS with a long retrieval suture via PTCD is safe and feasible for the treatment of cholangioenterostomy anastomotic stenosis.关键词
