内镜导丝网篮在ERCP治疗肝内胆管结石过程中的应用观察
Application of endoscopic guidewire basket during ERCP for the treatment of hepatolithiasis

Abstract
Endoscopic basket is the main instrument of peroral cholangioscopy during endoscopic retrograde cholangiopancreatography(ERCP)treating intrahepatic bile duct stones.Currently,the endoscopic basket used in clinical practice has many shortcomings,such as high insertion resistance,difficult release,and low success rate of stone extraction.We have designed a new endoscopic basket named"endoscopic guidewire basket"(ZL202521155283.7).The head of this endoscopic guidewire basket has a soft tip,which increases the passage of sheath tube,reduces the difficulty of basket release,and avoids the damage of bile duct mucosa caused by the head of ordinary endoscopic basket.A retrospective analysis was conducted on the clinical data of 8 patients who previously underwent ERCP combined with peroral cholangioscopy and failed stone removal with endoscopic basket,and then received re-stone removal with this endoscopic guidewire basket between October 2025 and December 2025 at East Hospital Affiliated to Tongji University.Among 8 patients,3 males and 5 females,aged from 38 to 72 years,with an average age of(64.25±9.25)years.For the stone removal failure reason of endoscopic basket,1 case of failing endoscopic basket insertion,2 cases of failing endoscopic basket release,and 5 cases of failing stone retrieval.Using this endoscopic guidewire basket inserted through the biliary forceps channel,we successfully removed intrahepatic bile duct stones in all 8 patients.The primary placement success rate of the endoscopic guidewire basket was 87.5%(7/8),with a placement satisfaction score of(7.75±1.98).The initial deployment success rate of the basket was 75.0%(6/8),with a deployment satisfaction score of(7.25±2.38).The first-attempt stone capture success rate reached 100%(8/8),with a stone retrieval satisfaction score of(8.25±1.28).The mean time of stone removal was(41.13±9.61)minutes,and the complete stone clearance rate in a single session was 75.0%(6/8).No biliary mucosal injury occurred during the procedure,and no postoperative complication such as cholangitis was observed.This study showed that our self-designed endoscopic guidewire basket is an effective tool for removing intrahepatic bile duct stones during ERCP.关键词
