子宫内膜癌患者术后早期应用低分子肝素对凝血-纤溶平衡及复发风险的影响研究
Effect of early postoperative low-molecular-weight heparin application on coagulation-fibrinolysis balance and recurrence risk in patients with endometrial cancer

Abstract
Objective:To investigate the regulatory effect of early postoperative low molecular weight heparin(LMWH)on coagula-tion-fibrinolysis balance and its impact on tumor recurrence risk in patients with endometrial cancer.Methods:A total of 110 patients with endometrial cancer who underwent radical surgery at Gansu Provincial Cancer Hospital from January 2022 to December 2025 were selected and randomly divided into an observation group(n=55)and a control group(n=55)using a random number table.The control group received conventional symptomatic supportive therapy postoperatively,while the observation group received subcutaneous injections of low-molecu-lar-weight heparin calcium 4100 IU once daily starting on postoperative day 1 for 10 consecutive days.Coagulation function parameters(PT,APTT,FIB),fibrinolysis parameter(D-dimer),and platelet count(PLT)were compared between the two groups before surgery and at 2 weeks postoperatively.The incidences of postoperative deep vein thrombosis(DVT)and bleeding events were recorded,and the postoperative tumor recurrence rate was analyzed at the 24-month follow-up.Results:At 2 weeks postoperatively,the D-dimer and FIB levels in the observation group were significantly lower than those in the control group(P<0.01).No significant differences were observed in PT,APTT,or PLT levels between the two groups(P＞0.05).The DVT incidence rate in the observation group was 3.77%,significantly lower than that in the control group(14.81%)(χ2=3.849,P=0.050).There was no significant difference in bleeding event rates between the two groups(5.66%vs.1.85%,P=0.360).The 24-month recurrence rates were also comparable between the two groups(12.73%vs.16.36%,P=0.590).Conclusions:Early postoperative administration of low-molecular-weight heparin in patients with endometrial cancer can effectively improve coagulation-fibrinolysis balance,and reduce the risk of postoperative deep vein thrombosis(DVT)without increasing the risk of bleeding events.However,it showed no signifi-cant impact on the tumor recurrence rate within 2 years after surgery.关键词
