再生障碍性贫血患者输血相关不良反应发生情况及影响因素研究
Occurrence of transfusion-related adverse reactions and their influencing factors in patients with aplastic anemia

Abstract
Objective To investigate the occurrence of transfusion-related adverse reactions and their influencing factors in pa-tients with aplastic anemia(AA),so as to provide a reference for clinical treatment.Methods Clinical data of 318 patients with AA admitted to Tangdu Hospital,Air Force Medical University,from January 2023 to December 2025 were retrospec-tively analyzed.According to whether transfusion-related adverse reactions occurred,patients were assigned to an occurrence group(n=30)and a non-occurrence group(n=288).Multivariate logistic regression analysis was used to analyze the influ-encing factors for transfusion-related adverse reactions.Receiver operating characteristic(ROC)curves were plotted,and the area under the curve(AUC)was used to evaluate the predictive performance of the influencing factors.Results Statisti-cally significant differences were noted between the two groups in age,number of transfusions,time from blood product re-lease from the blood bank to the start of infusion,fibrinogen(FIB)level,prothrombin time(PT),and maximum amplitude(MA)(P<0.05).Multivariate logistic regression analysis showed that number of transfusions>2 times,prolonged PT,and decreased MA were independent risk factors for transfusion-related adverse reactions(P<0.05).ROC curve analysis showed that the AUCs of number of transfusions,PT,and MA for predicting transfusion-related adverse reactions were 0.619,0.736,and 0.784,respectively.The combined AUC of these indicators was 0.858(95%CI:0.815-0.894),which was greater than that of any single indicator(P<0.05).Conclusion Allergic reactions are the most common type of trans-fusion-related adverse reactions in patients with AA.Number of transfusions,PT,and MA are closely associated with the occurrence of transfusion-related adverse reactions,and their combined use can effectively predict the risk of such reactions.关键词
